Obituary of Jacob Ray Sullivan

Jacob Ray Sullivan, age 25, of Lafayette, passed away Friday, December 11, 2015. Funeral Services for Jacob Sullivan will be conducted Tuesday, December 15, 2015 at 11 AM from the Chapel of Alexander Funeral Home with Elder Rick Jones officiating. Interment will follow in the Siloam Cemetery. Visitation with the family will begin Monday, December 14, at 10 AM and will continue until service time Tuesday at 11 AM. Jacob Sullivan is survived by; Son, Jeramiah Sullivan, Father, Phillip Sullivan and wife Kristyn, Mother, Tammy Sullivan, Brother, Jared Sullivan and wife Claudia, Sister, Makayla Sullivan, Paternal Grandmother, June Sullivan, Maternal Grandmother, Regina Strum, Nephew, Corbin Sullivan Nieces, Stella Sullivan, Brinlee Carver.
